Case Scenario - Nursing Care of the Patient with Heart Failure

Dosage and Calculations

Can you calculate dosages for:

1)            Flow Rates  - gtt/min, ml/hr

2)            Dosage Rates - mg/hr, mcg/hr, u/hr

3)            Dosages- mL, mg/dose

4)            Infusion Times ( in military time) - hr, min

5)            Dosage ranges

Study Guide-Dosage and Calculations

Calculate the infusion rate (gtt/min)

1.         D5W 2000 mL has been ordered to run 16 hour.  The calibration set is 10gtt/mL.

2.         An IV medication of 30 mL is to be administered over 30 minutes using a calibration set of

15 gtt/mL.

3.         D5 ½ with 20 mEq of KCl, 500 mL is ordered to run 25 mL/hr using a microdrip set.

4.         Infuse Gentamycin 100 mL via IVPB over 1 hour.  The calibration set is 10 gtt/mL.

Calculate the infusion time and completion time (in military time)

5.         A volume of 90 mL was started at 6:15 am and is to infuse at 30 gtt/min using a 20 gtt/mL set.

6.         A 20 gtt/mL set is used for a restart of 750 mL of D5W at 3:03 pm at a rate of 32 gtt/min.

7.         An infusion of 1000 mL of NaCl is to run 200 mL/hr.  It was started at 6:40 pm.

8.         An IV of 425 mL of D5 ¼ NS was restarted at 8:14 am and is to infuse at 15 gtt/minute using a 10 gtt/mL set.

Calculate the flow rate (mL/hr) in order to administer the following IV dosages.

9.         A 200 mg/hr dosage is ordered using a 100 mg/mL solution.

10.       A solution of 2 g of medication in 500 mL D5W is used to administer a dosage of 2 mg/min.

11.       A dosage of 2.5 g in 250 mL D5W has been ordered at a rate of 100 mcg/kg/min for an adult weighing 230 pounds.

12.       A 100 mcg/kg/min dosage has been order using a 5g/500 mL D5W solution.  The body weight is 77.6 kg.

13.       A post-op cardiac medication is infusing at 30 gtt/min.  The solution strength is 100 mg/500 mL D5W.  The calibration set is 20 gtt/mL.

a)               Calculate the mcg/min.

b)               Calculate mg/hr.

14.       A medication is to be titrated between 415-839 mcg/min.  The solution concentration is 100 mg in 40 mL.  Calculate the flow rate range.

15.       A 159 lb adult has a medication order to titrate at 3-6 mcg/kg/min.  The solution concentration is 50 mg in 250 mL.

a)               Calculate the dosage range.

b)               Calculate the flow rate.

If after several adjustments upwards the flow rate is stabilized at 75 mL/hr, what will the mcg/min dosage be?
