NCLEX type question Week of 8-15-10

A client with bipolar disorder is having difficulty sleeping. Which short-term goal is most appropriate for this client?

1. The client will develop a nighttime routine.
2. The client will exercise before bedtime.
3. The client will write a list of possible solutions.
4. The client will request medication for sleep.

  1. Answer 1

A nighttime routine or sleep ritual helps the client relax and prepare for sleep. Exercise may increase wakefulness. A list of solutions, although a good start, is not as effective as the implementation of a routine, and the routine is more appropriate than dependence on medication.
